An inquest into the deaths of Leicester City owner Vichai Srivaddhanaprabha and four others in a 2018 helicopter crash revealed that mechanical failure, specifically a seized tail rotor bearing, was the cause. The pilot, Eric Swaffer, is credited with attempting to control the helicopter despite the catastrophic failure.
