A woman underwent successful surgery to remove a rare and massive 10-kilogram phyllodes tumor from her breast. The case highlights the importance of early detection and seeking medical attention for any unusual breast changes.
Filomena Mauranik De Jesus initially opted for traditional remedies, applying herbal mixtures to the affected area, rather than seeking immediate medical attention. At first, she experienced no pain, leading her to believe it was a minor issue that didn't warrant further investigation. Her assumption proved incorrect as the lump grew progressively larger, eventually compelling her to seek medical help eight months later.
After blood tests and clinical examinations, doctors diagnosed the growth as a tumor, necessitating surgery to prevent more serious complications. \Before seeking professional medical help, Filomena tried drinking herbal concoctions for almost two years, hoping for a positive outcome. However, no improvement was observed during this period. In fact, two more new lumps appeared in the same breast in August and September 2025. The increasing size of the tumor negatively impacted her physical and emotional well-being, forcing the civil servant to take extended leave. The rapid growth of the lump within three months, starting in September, confined her to bed. Her movements were severely limited due to the 10-kilogram weight of the breast, especially given her overall body weight of only 42 kilograms. The breast frequently throbbed, became hot, and developed sores, requiring her to constantly apply wet towels to alleviate the pain. The condition made her physique resemble that of a pregnant woman, leading some to believe she was expecting a child. In November, Filomena, realizing the gravity of her condition, made the decision to return to the hospital. Doctors in Timor-Leste then arranged for surgery at the Prince Court Medical Centre. Accompanied by her sister-in-law, she arrived in Malaysia late last year to undergo surgery to remove the malignant phyllodes tumor. The complex four-hour procedure was led by a multidisciplinary team, including Consultant Breast and Oncoplastic Surgeon, Dr. Melissa Tan, Consultant Plastic and Reconstructive Surgeon, Dr. Kenneth Kok, and Consultant Hematologist, Dr. Muhd. Zanapiah Zakaria.\Phyllodes tumors are rare, accounting for approximately one percent of all breast tumor cases in adults globally. Statistics show that fewer than 30 cases of adolescents and children with this type of tumor have been reported worldwide. Dr. Melissa explained that phyllodes tumors are classified into three types: benign, borderline, and malignant. In Filomena's case, pre-surgical scans and examinations showed no evidence of metastatic disease, allowing the multidisciplinary team to proceed with definitive surgical treatment. The tumor had been present for nearly two years but grew rapidly in the last few months, significantly affecting the patient's mobility and daily life. Due to its size, the surgical team performed a mastectomy to ensure complete removal of the tumor. The 10-kilogram tumor, with a diameter of 46 centimeters, could potentially be one of the largest on record. All women, including children and adolescents, are potentially susceptible to phyllodes tumors, highlighting its ability to affect even young individuals. One of the oldest patients had a lump for 20 years without treatment until its size increased. This is because phyllodes tumors, similar to breast cancer, do not always present pain symptoms at the early stages. However, the exact cause of this disease remains unknown, although genetic and hormonal factors are believed to contribute to its growth. If surgery is not performed promptly, patients risk severe tissue damage, bleeding, infection, and tumor spread (metastasis). Therefore, Dr. Melissa emphasized the importance of early detection to enable timely treatment. She advised patients to be aware of any breast changes and to promptly consult a specialist for any rapidly growing lumps for further evaluation
