Queensland's mining and resource sector has a disturbing safety record - more than 55 deaths in the workplace in the last two decades. Today, a coroner handed down findings on just one of those cases – drill rig worker Gareth Dodunski who was only 21 when he was fatally crushed by machinery at a gas field in Western Queensland.

What makes this case extraordinary is the time Gareth Dodunski's family had to wait for the findings. Peter McCutcheon reports. In a statement to 7.30, Saxon Energy Services said: "The tragic events of 2013 remains in many peoples thoughts. Saxon Energy Services has fully cooperated over the past 10 years since the accident occurred, and following the company’s acquittal during court proceedings. At this stage, we are unable to provide any further information. The health and safety of our people is our first priority.
