This issue was seen as early as Harry Potter and the Chamber of Secrets and likely resulted from the limited technology available for special effects. However, the problem continued into later installments and has been prevalent even in the recent Fantastic Beasts films.

In the book, Harry and Draco threw as many spells as possible at each other.
