A Greek Ombudsman's investigation has concluded that the coast guard's inaction and failure to adhere to maritime protocol led to the sinking of a migrant boat off the coast of Pylos, resulting in the deaths of hundreds of people. The report recommends disciplinary action against eight coast guard officers and sheds light on a series of critical errors that contributed to the tragedy.

Greece 's Ombudsman has concluded that the coast guard failed to adhere to maritime regulations during one of the Mediterranean's most devastating shipwrecks in 2023. The findings, which have not yet been formally published, corroborate accounts from survivors and have been forwarded to a naval court investigating potential criminal negligence by the coast guard .

The inquiry, led by Greek Ombudsman Andreas Pottakis, recommends disciplinary action against eight coast guard officers for what it cites as 'clear indications' of dereliction of duty, putting the lives of those aboard the overcrowded fishing vessel named Adriana at risk. The Shipping Ministry has stated that judicial authorities will assess the report and that it maintains confidence in the coast guard's ability to 'effectively protect' Greek and EU borders.Meanwhile, coast guard authorities have steadfastly denied any wrongdoing and, when questioned for comment, referred to the Shipping Ministry's previous statement. The investigation revealed that the coast guard monitored Adriana for 15 hours before it capsized and sank in international waters off the southwestern town of Pylos on June 14, 2023. The trawler, carrying an estimated 750 people from Libya bound for Italy, tragically resulted in the deaths of most on board. Only 104 survivors are known to have made it ashore. The report details a series of concerning actions by the coast guard, including failing to follow proper protocol, delaying the search-and-rescue operation, and dismissing the boat's distress despite repeated calls for assistance from Frontex, the European Union's border agency. Furthermore, the report highlights the inadequate resources deployed to the scene, with only one coast guard vessel capable of carrying 36 people, equipped with special forces but limited rescue equipment. The Ombudsman's inquiry paints a grim picture of the events leading up to the disaster, raising serious questions about the coast guard's response and the potential for negligence that contributed to the tragic loss of life
