When there is a death or near death of a child in the child welfare system, the Department of Human Services and County CYS agencies are required to review the
Adams Co. CYS largely silent on unimplemented fatality report recommendations case, and DHS issues a report based on what happened. The Fatality reports detail if anything went wrong and if anything needs to be corrected. As part of that report, there are often recommendations that are offered by DHS and County agencies on how to improve systems moving forward.
In that report citations and recommendations are listed separately. DHS and Adams County made numerous recommendations, including the need for more intense parenting programs, a stronger system to assess supports in the family, ensuring there is a process for caseworkers to evaluate a parent's learning and recommending that service providers are included in the decision-making process regarding reunification, among others.
In a statement, a spokesperson for the Department of Human Services said, "DHS remains committed to being a partner to County Children and Youth Agencies . Following the investigation and the finalization of a child fatality report, DHS' Office of Children, Youth, and Families utilizes the regular monthly check-in meetings that occur with the county as an opportunity to review recommendations that have been suggested or implemented as a result of a child fatality.
